Government policy (Health Department) about Polindes, that is continue steps for program that has aim to provide midwifes at villages since Pelita V (1989/1990). The aim, beside of effort to provide healthy service infrastructures at villages, it is decreasing sickness and mortality number for pregnant and birthing woman, and to give information and go with them about understanding daily needs in healthy field.

This research takes a picture about the implementation of Polindes program at nine villages at three sub district at Agam regency West Sumatera. From field data, there is found three problems: first, there is low realization for physical infrastructures like building for Polindes and it is without propagation, which is available with concentrate. Second, there is low qualification for Polindes, and third, there is function, which is not going ideally of societies. This thing, it is marked with case there are not coordination and crossing-sectors education from the medical non technical aspect, and there is no working line between midwifes at villages and indigenous medical practitioner for baby births.

The embrace of birthing help activities by midwifes at village get the number of 40 per cent from 78.64 per cent birthing that is helped by medical professionals and most of the birthing, it is happened at patients’ house not at Polindes. There is unwilling by patients birthing at Polindes not only there is not support of technical infrastructures but also because of patient do not find personal home situations and birthing experiences that is close at Polindes. For most of village societies, birthing moment and birthing it self are culture social tendency where families and societies will get apart for it. This condition is crossing with Polindes institution where pregnant and birthing fully is medical tendency. It is entire obstetric step sequences is procedural and de-personal with the arbiter authority at midwifes hand. This phenomenon with lost of authority and family support for pregnant and birthing moments.
