Maternal mortality rate is still high assumed to be related with the high percentage of TBAs (traditional birth attendants) deliveries. To overcome this situation the government has planned to place midwives at all villages. This study was conducted at Tulung Agung regency, Sendang and Besuki subdistricts and each subdistricts three villages where there is a midwife as well as TBAs were selected.

In total six TBAs and six midwives at the villages were interviewed. A household survey was carried out interview to mothers who have under fives. Focus Group Discussion were done to get information from community leaders, mothers who delivery assisted by TBA and assisted by midwife. The results showed that there was a marked difference of deliveries assisted by midwives and TBAs; in Sendang subdistricts (midwives 65.9 per cent, TBAs 34.1 per cent). The significant correlation factors of deliveries assistant selection were education, economic status, health knowledge and attitude and behaviour to the tradition which link to pregnancies, deliveries and post deliveries.The role of TBAs are still high especially postnatal care for mothers and her babies eventhough the deliveries were assisted by midwife. Prenatal care is provided by the midwives but it is still common that the deliveries are assisted by TBAs.

So far two models of delivery service are discovered, assisted by midwife or TBA and assisted by both of them. Most of deliveries are usually conducted at home or at the hospital if referral is needed. The utilization of the villages home delivery (Pondok Bersalin Desa) as a place of delivery is not happening yet. It is suggested that deliveries are assisted by midwives and TBA, where the TBA assist the midwife as an integrated team. To achieve it, guidence and good trainings to midwives and TBAs are needed.
